The Hospice Bereavement Coordinator is responsible for Bereavement Counseling and implementing the Hospice Bereavement Program by providing grief support to identified survivors according to the established plan of care.


 


Please note: This is a part time (20 hour per week) position. The position may require travel throughout our hospice service area to meet with bereaved families and/or caregivers.


 


Primary Responsibilities of our Hospice Bereavement Coordinator: 



  • Providing support to the patient, family and an array of individuals needing care for issues related to grief, loss and adjustment prior to the death and at the discretion of the Interdisciplinary Group (IDG)

  • Participation in our Vigil program

  • Contributing to the individual plan of care as necessary to reflect pre-death involvement and ensure on-going assessment of these needs

  • Developing and initiating the Bereavement Contact Plan in collaboration with the IDG members after the death

  • Leading site specific interdisciplinary team meeting ritual for memorializing patient served and facilitates discussion of survivor risk factors to complicated grief outcomes

  • Conducting bereavement assessment to assess grief reactions and coping skills in order to establish bereavement plan of care

  • Providing bereavement services as outlined in the bereavement plan of care for up to 13 months after the death

  • Referring families/patients to appropriate community resources

  • Extending bereavement support to residents or staff of a skilled nursing facility/nursing facility when appropriate

  • Participating in the selection, training, assignment and supervision of dedicated bereavement volunteers in agreement with the Volunteer Coordinator

  • Orchestrating memorial events and developing bereavement services as needed

  • Overseeing the outcome of the bereavement programs based upon the needs of the patient and family

  • Providing educational programs and materials for patients/families, staff and the community on loss, grief and coping with bereavement

  • Participating in community outreach and education activities as necessary to support needs of individual hospice site

  • Establishing therapeutic relationships with clients from a variety of cultural, religious/spiritual and socio-economic backgrounds

Requirements

Required Qualifications:



  • Bachelor's degree in social work, psychology or equivalent theological degree from college or university, or in another closely related field or certification of professional level training in bereavement

  • Clinical experience in the areas of life-threatening and chronic illness

  • Ability to identify clients at risk for complicated grief

  • Ability to recognize sign and symptoms of chemical dependence and other psychiatric illnesses

  • Excellent written and oral communication

  • Reliable transportation including valid driver's license and auto insurance

  • Computer knowledge and experience

 


Preferred Qualifications:



  • Master's degree

  • Experience leading individual and/or family grief support groups

  • Hospice experience
