Our full time on call position offers the opportunity to work on our team of compassionate NPs to ensure continuity of care for our members. This is a work from home position with occasional onsite visits/trainings. Schedule varies and will help with covering off hours Monday through Friday 6:00pm to 8:00am, weekends and holidays.



In this role, you will provide on call medical management for patients in all lines of business. You will collaborate with nursing home staff, PCPs and responsible parties on plans of care, and utilize electronic medical records for documentation.

Primary Responsibilities:




  • Full Time Nurse Practitioner On Call coverage telephonic and onsite.
  • This position will support all four products in Wisconsin
  • Candidate will assume the role of a dedicated on-call position
  • Will work from home and cover incoming calls for SNF, ALF, members, families
  • Willing to work off hours, be a strong team player, understand business goals and ability to navigate healthcare systems safely and efficiently
  • Excellent communicator with a strong desire to help others. Informal leader to SNF/ALF partners on the off

Requirements

Required Qualifications: 



  • Current, unrestricted NP license in Wisconsin
  • Unrestricted DEA (license to prescribe narcotics/level II info here) per state regulations (unless prohibited in state of practice) or the ability to obtain
  • Nurse Practitioner national certification as ANP, FNP, or GNP required
  • A clinical background in adult, family or geriatric specialties 
  • Past experience working in a nursing home or with seniors in other settings desired 
  • Intermediate level of proficiency in PC based word processing including Microsoft Word, Outlook and EMR
  • Proficient computer skills including the ability to document medical information with written and electronic medical records
  • Understanding of Geriatrics and Chronic Illness
  • Ability to develop and maintain positive customer relationships
  • Adaptability to change
  • Understanding of Advanced Illness and end of life discussions
