The Associate Director Clinical Outreach Manager is responsible for developing and maintaining relationships with the external agencies that Optum Oregon will partner with to coordinate care related to Care Management Programs and services.


 


Primary Responsibilities:



  • The Associate Director Clinical Outreach manager is our liaison with providers and referral agencies and also oversee our provider performance monitoring.

  • Participates on QC/PI Committee (QC/PIC)

  • Meet with providers, provider organizations, advocacy groups and other community organizations to explain the program's services and provide contact information

  • Our Provider Outreach Manager's outreach will become one of the most valuable steps in the early identification of Members who need assistance

  • Assist providers in using the web-enabled clinical record, referring Members for care coordination, or requesting additional support from the program


  • Coordinate and influence superior customer service delivery by developing and maintaining strong intercompany relationships



  • Maintain a high level of healthcare industry knowledge as well as customer's industry




  • Establish and maintain ''strong and appropriate' level relationships with FFS Provider network




  • Communication and ability to network with different organizations agencies




  • Reports to Executive Director


 

Requirements

Required Qualifications:



  • Licensed Clinician in the State of Oregon-MSW or RN

  • Experience in public sector, such as Medicaid and Medicare

  • Experience in marketing community service delivery, or advocacy

  • Knowledge of Oregon healthcare, public health infrastructure and resources

  • 5 years of experience handling complex and difficult accounts

  • 5 years of experience working in partnership with providers; specialty offices; community and non-profit organizations

  • 5 years of experience working in healthcare delivery system


  • Great presentation skills
