The purpose of this position is to provide support to the Case Management Program by delivering service telephonically. Job functions include assisting with member care coordination and community resources integration. Responsible at a central office to assist in triage and coordination supporting UHC M&V Care Managers by assisting with eligibility determination, planning coordination, education and delivery of case management services, engaging with member, families or other health care team members as directed.

As a potential government contractor, this position requires U.S. citizenship and proof of favorable adjudication following submission of Department of Defense form SF85P.

Primary Responsibilities:
  • Identify, engage, educate and coordinate case management services
  • Assist with the identification of national resources to support community based activities, coordinate and educate referral sources within the UHG enterprise
  • Responsible  to assist in triage and coordination supporting UHC M&V  Case  Managers
  • Assisting with determining eligibility, planning, coordination, education and delivery of case management for the appropriate members
  • Engaging with member, family or other health care team members as directed.
  • Maintains accurate documentation systems/data base which assures compliance with program process and objectives and reporting

Requirements

Required Qualifications:
  • High School Diploma/GED.
  • 1+ year of Customer Service experience.
  • 1+ year of experience using the telephone and computer as the primary instruments to perform the job duties.
  • Experience working within the Healthcare Industry.
  • Experience using a computer and Microsoft Office (MS Word, MS Excel, and MS Outlook)
  • United States Citizenship.
  • Ability to obtain favorable adjudication following submission of Department of Defense form SF86 NAC form SF86.
Preferred Qualifications:
  • Associate's Degree or Bachelor's Degree.
  • Working knowledge of Medical Terminology.
  • Familiarity with TRICARE.
  • Experience working with Medicare and/or Medicaid Services.
  • Professional experience in a Clerical or Administrative support related role.
