Primary Responsibilities:
- Identify, engage, educate and coordinate case management services
- Assist with the identification of national resources to support community based activities, coordinate and educate referral sources within the UHG enterprise
- Responsible to assist in triage and coordination supporting UHC M&V Case Managers
- Assisting with determining eligibility, planning, coordination, education and delivery of case management for the appropriate members
- Engaging with member, family or other health care team members as directed.
- Maintains accurate documentation systems/data base which assures compliance with program process and objectives and reporting
Requirements
- High School Diploma/GED.
- 1+ year of Customer Service experience.
- 1+ year of experience using the telephone and computer as the primary instruments to perform the job duties.
- Experience working within the Healthcare Industry.
- Experience using a computer and Microsoft Office (MS Word, MS Excel, and MS Outlook)
- United States Citizenship.
- Ability to obtain favorable adjudication following submission of Department of Defense form SF86 NAC form SF86.
- Associate's Degree or Bachelor's Degree.
- Working knowledge of Medical Terminology.
- Familiarity with TRICARE.
- Experience working with Medicare and/or Medicaid Services.
- Professional experience in a Clerical or Administrative support related role.
