Objective:

Perform a variety of duties in the area of client services and care management.  The Care Manager is expected to oversee and coordinate the care of our senior clients to provide the highest quality service and allow them to remain independent in their homes.  This position requires an individual who thrives on substantial person-to-person contact during the business day. Our services are in high demand and you must be able to thrive in a fast paced environment. You will not thrive with us if you are unable to work in a team environment.

Primary Responsibilities:

•    Develop and maintain positive relationships with all clients through phone calls, cards, and special events
•    Utilize the CAREGiver Productivity Report to ensure that appropriate usage of CAREGivers is achieved
•    Work as a team with co-workers, the Care Management Team Leader, Client Service Representative and Training Coordinator to ensure that clients are receiving the highest quality of care.  
•    Communicate identified concerns and health declines to the direct Client Service Team Member and/or the Care Management Team Leader
•    Collect and report care management department’s statistics for weekly departmental meeting
•    Report quality assurance concerns to Client Services Representative and/or Care Management Team Leader
•    Monitor Telephony system for late clock ins and lack of clock ins and call CAREGiver and/or Clients as indicated
•    Coordinate and maintain monthly client schedules in operating software
•    Enter and maintain accurate client and CAREGiver records in operating software
•    Establish and maintain a personal, yet professional relationships with clients, families and families
•    Perform weekend office hours and afterhours on-call Care Manager duties as assigned

Secondary Responsibilities:

•    Correspond with clients on holidays and birthdays
•    Participate in CAREGiver meetings
•    Perform any and all other functions and responsibilities deemed necessary

Education/Experience Requirements:

•    High school graduation or the equivalent. 
•    Five years of related business experience or an equivalent combination of education and work experience may be considered.
•    Must possess a valid driver’s license.

Knowledge, Skills and Abilities:

•    This position must have an understanding of and uphold the policies and procedures established by LA Homecare, LLC, d.b.a. an independently owned and operated Home Instead Senior Care franchise.
•    This position must demonstrate excellent oral and written communication skills and the ability to listen effectively. 
•    This position must have the ability to work independently, maintain confidentiality of information and meet deadlines. 
•    Knowledge of the senior care industry.
•    Ability to demonstrate effective interpersonal skills essential as well as sound judgment and good decision-making skills. Must demonstrate discretion, integrity and fair-mindedness consistent with company standards, practices, policies and procedures.
•    Ability to organize and prioritize daily, monthly and yearly work. 
•    Ability to establish good working relationships with management, colleagues, franchise owners and their staff.
•    Ability to sit at a desk and listen effectively for long periods of time on the telephone.
•    Ability to stand and walk during the course of a normal business day.  
•    Ability to present a professional appearance and demeanor.
•    Must be patient and congenial on the telephone.  
•    Must have computer skills and be proficient in Word and Excel. 
•    Ability to work evenings or weekends as required.
•    Ability to perform duties in a professional office setting.
