Join a newly created specialty Informatics team that designs and implements a revolutionary, innovative health care record to support health services programs, including, High Risk Case Management, Congestive Heart Failure, Diabetes and Advanced Illness for Commercial, Medicare, Medicaid, and Community & State programs. Our mission is to provide a state of the art documentation record that enhances clinical services to improve patient outcomes.
The Clinical Informatics Specialist will act as a clinical Subject Matter Expert and be responsible for the development and support of the standardized data needed for documentation of clinical services to the innovative documentation record. Will utilize clinical expertise to act as a translator and advocate guiding the implementation of standardized language into the health care record to measure and analyze clinical data.
Primary Responsibilities:
- Act as a nursing and health informatics expert supporting the clinical teams, case managers, clinical training, Information technology and Healthcare Analytics, translating the clinical processes into Assessment, Pathway, and Plan of Care tools.
- Recognizes opportunities to maximize technology to enhance individual and population health outcomes, efficiency, and clinical documentation that clearly tells the defined Product/Program value story.
Designs processes within the Plan of Care capability to collect, record, and share consumer information, reduce duplication, standardize clinical practice and documentation, and meet all regulatory requirements. This includes:
- Mapping clinical data to standard taxonomy as translation expert
- Develop clinical guidelines from evidenced based practice
- Work with Nurses as subject matter expert in standard terminologies
- Support construction and maintenance of data definitions for system use and interpretation
- Support mapping of clinical assessments to standard data structure
- Act as project lead for implementation and migration of clinical programs
- Utilizes critical analysis to plan, problem solve, and to mitigate risk in the deployment and implementation of an innovative health care record.
- Assure reporting and analytics metrics are addressed in data collection available from the electronic health record.
- Partner with the value realization team, assure program value metrics are defined and met by the standardized documentation.
- Problem solves application and workflow issues, works with team to resolve the concerns and improve results to accurate reflect clinical services.
- Develops and updates policies, procedures, and job aids pertaining to clinical information system management.
Requirements
Required Qualifications:
- Bachelor's Degree in Nursing or other Healthcare related degree
- Clinical Informatics coursework
- 3+ years of experience in a clinical setting
- Strong business management or project management experience
- 1+ year of clinical informatics experience (work or project related)
- Demonstrated experience with standardized languages (working knowledge of Omaha System)
- Proficiency with Microsoft Office programs Excel and PowerPoint
Preferred Qualifications:
- Master's Degree or Graduate Study in progress
- A valid, unrestricted and current RN license
- 3+ years of clinical experience in Case management or Home Health
- Managed Care background and/or experience
- Proficiency with Microsoft Office programs Excel and PowerPoint and Access
