POSITION SUMMARY : To rotate mainly in ED. Shifts initally 11 a.m. to 7 p.m. The Staff Pharmacist is responsible for the procurement, distribution, control and record keeping of pharmaceuticals within the hospital. Duties include the direct supervision of the pharmacy technician in the operational functions of the pharmacy. Staff Pharmacist duties will be carried out in accordance with all state and federal laws, national regulatory agencies and the facility departmental policies and procedures.

SPECIFIC ELEMENTS & ESSENTIAL FUNCTIONS :

• Adheres to all Board of Pharmacy, JCAHO, CMS, DEA and FDA standards

• Order entry, performs pharmacist check of packaged and bar-coded pharmaceuticals, and oversight of the distribution process

• Completion of orders in the Problem Queue or Follow up Queue

• Ensures that all technician duties have been completed

• Review clinical reports including: renal dosing, IV to PO, Antibiotic Stewardship, Therapeutic Drug Levels, Abnormal lab values

• Follow up on urgent patient care issues as needed

• Follow up on pending interventions

• Follow up on dosing consults (i.e. pharmacodynamics, kinetics)

• ICU rounds: attend on a daily basis when Critical Pharmacist not available; at minimum, review patient profiles daily (seven days a week)

• Non-critical care areas: ideal, review all patient profiles daily; minimal, all new admissions

• Antibiotic Stewardship (at minimum, review targeted antibiotics and positive cultures)

• Medication Reconciliation

• Anticoagulation Therapy Management (at minimum, review to ensure compliance with hospital policy and regulatory standards)

• ADR monitoring / reporting

• Medication Use evaluation

• Medication event reporting (actual and close calls)

• Interpersonal skills, effective communication and presentation skills, able to work collaboratively with co-workers, other healthcare professionals

• Represent pharmacy department on various committees

• Policy and procedure development

• Miscellaneous clinical projects

• Follow all safety policies and procedures

• Meet all department mandated timeframes and guidelines

• Practice and adherence to the Code of Conduct philosophy and Mission and Value Statement

• Attend all required training and education set by the organization or pharmacy department

• Perform other duties as assigned

Recent hospital experience is required. Post graduate degree preferred. Pharmacist license also required.
