GENERAL SUMMARY OF DUTIES: Evaluates patients and will implement, plan, and modify treatment programs according to age appropriateness. Participates in D/C planning, performs administrative duties, participates in QI, pursues professional growth, assists with instruction and education of PT aides/students/volunteers, and performs other duties as assigned. On occasion, transfer to other sites may be necessary.

Duties include but not limited to:

1. Evaluates patient musculoskeletal and neuromuscular systems, recognizing functional impairments, abilities, and psychosocial influences.

2. Assesses and re-examines the physical, functional, perceptional-motor, psychosocial, and vocational status of patients. Evaluates prognosis for recovery. Demonstrates use of proper body mechanics.

3. Establishes an appropriate therapeutic program and integrates that program with the patient and family’s overall care plan and goals.

4. Implements and revises the individualized treatment program in coordination with the interdisciplinary treatment plan and documents according guidelines.

5. Participates in discharge planning and develops follow-up programs as indicated.

6. Communicates effectively both in written and verbal form with patients, families, physicians, payers, rehabilitation staff, and other allied health professionals.

7. Completes adjunct patient care functions such as: medical record documentation, scheduling, productivity information, and service charges in accordance with regulatory agencies and department standards.

8. Pursues and facilitates professional and department growth through continuing education and in-service programs.

9. Demonstrates good working knowledge of treatment techniques and is creative with treatment approaches; utilizes available community resources to augment treatment program.

10. Exhibits professional behavior that promotes a working environment of efficiency, productivity, and cooperation.

11. Must read, understand, and adhere to all HealthONE Physician Care policies and procedures.

12. Practice and adhere to the Code of Conduct and Mission and Value Statement.

KNOWLEDGE, SKILLS & ABILITIES – This position requires the following minimal requirements:

Able to perform all evaluations, assessments, and treatment planning in this position.

Oversees work of P.T. assistants, aides, volunteers, and students.

Must have good interpersonal and organizational skills.

Ability to communicate effectively both verbally and in writing.

Must be flexible and able to handle multiple tasks.

Word processing experience preferred.

EDUCATION

Master’s in Physical Therapy

EXPERIENCE

Zero to three years depending on job requirement.

CERTIFICATE/LICENSE

Current licensure by state of CO as P.T

BLS certification
