Bilingual Master of Social worker needed for a Full Time/Contract opportunity with Yoh's client located in Los Angeles, CA.
Top Skills Should You Possess:
- Master of Social Worker (MSW) in State of CA (Active and in Good Standing)
- Must maintain current CPR/First Aid Training
- Must be Bilingual in Spanish
- 2-5 years of Case Management experience, specifically creating care plans; knowledge of physical, mental and social needs of the elderly
What You'll Be Doing:
- Responsible for Utilization Management, Quality Referrals, and Discharge planning in a Pace Elderly Setting
- Demonstrate the knowledge and skills necessary to provide appropriate case management
- Perform psycho-social assessments. Contact family and past mental health providers for histories
Assess discharge-planning needs of a diverse cultural and ethnic population - Assigned patients ranging Adults to geriatrics- must have experience
- Must be flexible between hours of 8am-8pm
What You Need to Bring to the Table:
- Master of Social Worker (MSW) in State of CA (Active and in Good Standing)
- or BSW with 2 years' experience
- 2-5 years of Case Management experience, specifically creating care plans; knowledge of physical, mental and social needs of the elderly.
- Must have 3-5 years of Clinical Experience
- Bilingual in Spanish is a Must
- Knowledge of DHCS, OSHA, safety regulations and HIPAA Guidelines including the PHI privacy requirements
- Intermediate level use of MS Word, Excel and Outlook
- Strong communication and interpersonal skills
