REM has partnered with the Minnesota Department of Human Services to participate in the Transitions to Community Initiative to develop unique discharge plans to individuals currently at AMRTC or MSH-St. Peter who are unable to be discharged due to individually-specific barriers. The identified individuals at AMRTC or MSH-St. Peter have complex medical and/or behavioral health care and/or socio-emotional needs that complicate discharge planning which may include, but are not limited to, co-occurring substance, physical, or cognitive disorders, developmental disabilities, aggressive behaviors, high acuity of symptomatology, refractory mental health symptoms, traumatic brain injuries, Mentally Ill and Dangerous (MI& D) status, or other legal issues that have led to the need for a high level of intensive services that cannot be met by current community services. In 2014 REM was awarded a grant titled “the whatever it takes” grant to develop resources to assist these individuals transitioning from state hospitals to housing and services of their choice that meet their health and safety needs. The goal of the grant is to lengthen the time the individual spends integrated into the community and reduce hospitalization through the provision of individualized services. One of the unique aspects of this initiative is that the individual receiving services takes an active role in creating a discharge plan that they think will work for them. We as a provider are given the task of doing “whatever it takes” to help make this individual have a successful and long term community placement.

Program Coordinators are a critical link to the community for those we serve, as well as serving as a bridge between our direct care and clinical staff to ensure the seamless delivery of services to the individuals we support.

REM is seeking a Program Coordinator to lead a team of caregivers on site in providing and managing the direct care of our customers in a group home setting. Program Coordinators facilitate the implementation of Individual Service Plans (ISP) and participate in a range of administrative duties including staff hiring, training, scheduling and oversight.

In addition, Program Coordinators may provide support to individuals receiving services, including: assistance with daily activities such as meal preparation and planning; medication administration; and personal care. Program Coordinators may also facilitate community involvement by accompanying individuals on outings in the community or providing job training and transportation to work, therapy, medical appointments or other activities.

Qualifications

Requirements:

  • High school diploma or GED
  • Two years related work experience is preferred
  • Must be 18 years of age
  • Valid driver’s license in good standing
  • Car registration and vehicle insurance if providing transportation for individuals receiving services
  • Successful clearance of local and/or state background checks
  • Six months of professional experience in working with individuals living with a mental health diagnosis or willingness to learn how to successfully support our customers
  • Willingness to support individuals in an environment that promotes dignity and respect
  • Supervisory experience preferred
