Primary Responsibilities:
- Engaging high risk members in person (in SNF's/nursing homes) to conduct assessments, and coordinate services and community resources
- Functioning as an advocate for members and decreasing unnecessary hospitalizations and emergency room visits
- Travel in assigned territory as needed to complete patient assessments; the rest of the time will be spent working in a home-based setting (communicating telephonically with hospitals or post SNF providers)
- Delivering a holistic approach to coordinated care based on the enrollee needs
- Identifying early risk factors and conducting ongoing assessments and documenting in an electronic file
- Collaborating with Nurse Practitioners and Physician Assistants to coordinate necessary services
- Building relationships with NP's and PA's, and other SNF staff members
- Ensuring cases are documented in a timely manner
Requirements
Required Qualifications:
- Active, unrestricted RN license in the state of North Carolina
- 3+ years of clinical experience in any setting
- Level of proficiency with Microsoft Office, with the ability to navigate a Windows environment
- Strong organizational skills and multitasking abilities
- Ability to work independently
- Strong patient assessment skills are a must
- Reliable vehicle with the appropriate insurance, and the ability/willingness to travel within the service delivery area (greater Charlotte, NC area)
Preferred Qualifications:
- Bachelor's Degree in Nursing
- Certified Case Manager (CCM)
- Prior experience working for an insurance company or other managed care setting in case management, utilization review, discharge planning, etc.
- Previous experience with the Medicare population
- Home Health experience
- Experience working in a nursing home or Skilled Nursing Facility
