Primary Responsibilities:



  • Engaging high risk members in person (in SNF's/nursing homes) to conduct assessments, and coordinate services and community resources 

  • Functioning as an advocate for members and decreasing unnecessary hospitalizations and emergency room visits 

  • Travel in assigned territory as needed to complete patient assessments; the rest of the time will be spent working in a home-based setting (communicating telephonically with hospitals or post SNF providers) 

  • Delivering a holistic approach to coordinated care based on the enrollee needs 

  • Identifying early risk factors and conducting ongoing assessments and documenting in an electronic file 

  • Collaborating with Nurse Practitioners and Physician Assistants to coordinate necessary services 

  • Building relationships with NP's and PA's, and other SNF staff members 

  • Ensuring cases are documented in a timely manner

Requirements

Required Qualifications:



  • Active, unrestricted RN license in the state of North Carolina 

  • 3+ years of clinical experience in any setting 

  • Level of proficiency with Microsoft Office, with the ability to navigate a Windows environment 

  • Strong organizational skills and multitasking abilities 

  • Ability to work independently 

  • Strong patient assessment skills are a must 

  • Reliable vehicle with the appropriate insurance, and the ability/willingness to travel within the service delivery area (greater Charlotte, NC area)

 Preferred Qualifications:



  • Bachelor's Degree in Nursing 

  • Certified Case Manager (CCM) 

  • Prior experience working for an insurance company or other managed care setting in case management, utilization review, discharge planning, etc. 

  • Previous experience with the Medicare population

  • Home Health experience

  • Experience working in a nursing home or Skilled Nursing Facility
