Job Duties Include: Under general direction, serves as a liaison and resource to referral sources. Coordinates home care services to patients and works as an advocate to improve physician referral rates. Coordinates the dissemination of information from physicians to the WIRS office. Provides referral sources with ongoing training, information regarding changes in programs and services, and orders and distributes service information. Interacts with physicians and hospital systems on a daily basis to provide an overview of services for patients who have been referred to WIRS. Serves as a point of contact, coordination, and communication with other providers. Coordinates service requests with the office and the patient. Attends physician office manager events to provide training and conduct presentations.

Communicates timely with the discharge planner/ social worker to coordinate needed Home Care services on patients that have been referred to WIRS. Maintains information on WIRS’s patients that have been hospitalized and communicates back to the manager. Maintains relationships with referral sources and serves as a liaison between the hospital and Home Care setting. Helps to orient the sales staff to the physician base and their offices and operations. Effectively communicates with agency staff, medical team, patients, and family throughout the discharge process. Works in collaboration with the office, clinical staff, and the patient and his/her family members or caregivers. Reviews the patient’s medical record to obtain primary and secondary insurance payor information, and relays this to the intake department. Notifies Managers of discharges in a timely manner. Makes arrangements for any special medical supplies or appliances to be available.

Effectively communicates with marketing staff and development of educational and promotional programs. Maintains Confidentiality of patient and proprietary information. Observes legal and ethical guidelines for safeguarding the confidentiality of patient and proprietary WIRS information. Participates in and attends required in-services. Demonstrates current knowledge regarding the care and management of patients in the alternate site. Completes all WIRS educational modules assigned by supervisor. Obtains all educational requirements necessary to maintain professional license. Participates in WIRS’s Performance Improvement Program. Contributes to the achievement of WIRS’s goals through effectively participating in committees and or informal work groups. Identifies opportunities for improvement and communicates to appropriate staff. In all activities, complies with accreditation, legal, regulatory, and safety requirements. Participates in the nursing department on-call responsibilities.

Minimum Requirements:

High School diploma and 2 years of healthcare experience

Experience using clinical terminology

Experience in providing training and developing process documentation/user manuals

Experience in coaching, mentoring, and training staff

Experience using time management skills such as prioritizing/organizing and tracking details and meeting deadlines of multiple projects with varying completion dates

Experience establishing & maintaining relationships with individuals at all levels of the organization, in the business community & with vendors

Preferred Qualifications:

Bachelor’s or Associates Degree in healthcare administration or related field

Licensed as a Pharmacy Technician, Dietitian, or Respiratory Therapist in the state of residence
