About this job

Working with health plan members and the Social Security Administration (SSA) to determine and secure disability determination for the member and increased payment capitation for the plan; develop and maintain positive, productive relationships with health plan members, SSA staff, and medical providers.

*Performs post initial application denial rescreening for appropriateness to take to appeal/recon

*Secures authorizations to represent plan member before both SSA and State / County medical assistance offices.

*Assists plan member in acquiring verifications; helps secure forms, fill out forms; gets forms to proper agency; follows up with plan
member and agencies to ensure processing

*Completes required state and federal forms as documentation of plan members medical condition

*Determines / secures details regarding appropriate historical medical providers for documentation of complete medical history

*Maintains regular contact with plan
member throughout all processes

*Informs plan member of community resources/makes referrals to agencies
Issues correspondence to plan members

QUALIFICATIONS (MUST HAVES IN ORDER TO BE CONSIDERED)

6 months+years experience in medicaid or medicare case management.
