Job Description:

Essential Duties and Responsibilities 1. Provides supportive services to enhance treatment goals and effectiveness. 2. Assists members in securing a variety of social services, including but not limited to, relapse prevention services, prenatal care, medical care, food stamps, general assistance and child-care. 3. Communicates and coordinates care with the member's family, behavioral health and general medical and dental health care providers, community resources, and other involved supports including educational, social judicial, community and other State agencies. 4. Provides outreach and follow-up of crisis contacts and missed appointments. 5. Participates in staffing's, case conferences or other meetings, as appropriate. 6. Transports members to and from appointments when appropriate. 7. Provides community and home-based services to members, when appropriate. 8. Monitors level and intensity of services contained in the members' individual service plans. 9. Maintains member records in accordance with agency and funding source requirements. 10. Maintains regular contact with treatment staff to inform and be informed of member's progress and follows recommendations for continuing care. 11. Meets with service providers to review utilization and monitor member progress towards program goals. 12. Provides member advocacy, as needed, to ensure delivery of required services. 13. Provides community outreach and education regarding program services. 14. Provides crisis intervention to include location of appropriate providers of service. 15. When appropriate develops transitional service plan with members, prior to reentry into the community. 16. Performs other related duties in accordance with agency growth and changes

Work Environment:

Very challenging environment. Candidates will be dealing with clients with serious behavioral issues. Must be prepared for anything.

Qualifications:

Bachelor's Degree in Behavioral Health/Psychology/Sociology or other related fields 0-3 years of behavioral Health Experience Must pass the Skills Test

Crisis Intervention Techniques 1. Effectively demonstrates de-escalation of agitated members 2. Reacts timely and logically in crisis 3. Appropriately formulates Crisis Treatment Goals and Objectives. 4. Appropriately demonstrates knowledge of and implements Petitioning Procedures Active Treatment 5. Effectively provides or ensures that active treatment is provided to assigned members. 6. Effectively develops, implements, and revises service plans based on continuous assessment of the member. 7. Demonstrates effective listening skills 8. Demonstrates effective summarization skills 9. Demonstrates effective forming of a therapeutic relationship 10. Effectively engages member in treatment Case Management 11. Demonstrates knowledge of community resources 12. Demonstrates competency forming relationships and coordinating with other agencies 13. Demonstrates respect and caring to members

Additional Compensation:

$1 shift differential for any hours worked before 7 AM and after 7 PM. Overtime will be worked and it is a non-exempt position
