This position is onsite at an Aetna client based out of Hickory, NC.
Candidates must have at least 3 years of clinical nursing experience. Candidates that have experience with motivational interviewing/coaching is strongly preferred.
POSITION SUMMARY
The Health Coach Consultant utilizes a collaborative process of assessment, planning, implementation and evaluation, to engage, educate, and promote/influence members decisions related to achieving and maintaining optimal health status. Requires an RN with unrestricted active license.
We are looking for a Registered Nurse to work in a on-site facility in Hickory North Carolina . You will play a pivitol role in the Wellness team through coaching and developing effective care coordination and self care for members who are actively participating in our Healthy Lifestyle Coaching and Disease Management program.
The ideal candidate will possess the many skills necessary to expertly assist members in identifying modifiable risk factors and coaching on lifestyle changes necessary to reduce risk. Disease Management responsibilities will also include assessing members needs for chronic health conditions and developing and implementing goals with member to to maintain optimum level of health. Collaboration with Healthy Lifestyle Coaching team and Disease Management teams is an integral part of this position. In addition, this role will develop and provide occasional educational presentations members.
Education/Licenses and Certifications: Graduate of an accredited nursing program and currently licensed as a Registered Nurse in the State of North Carolina. Minimum 3 years of recent experience. Strong knowledge of major health behavior models including Motivational Interviewing. Experience in weight management. Possess strong verbal, written communication skills, computer and keyboarding skills. This is a 40 hour per week position, candidate must be flexible in scheduling to meet business needs.

The highest level of education desired for candidates in this position is a Bachelor's degree or equivalent experience.
