CareMore's Advanced Practice Clinicians (i.e. Advanced Practice Registered Nurses (APRNs), Nurse Practitioners (NPs), Physician Assistant (PAs), etc.) support our patients by providing exceptional care in our care centers and other care environments. They work interdependently with physicians and allied health professionals to coordinate care to enhance the quality of life of the patient.

Essential duties and responsibilities include but are not limited to:

Develops and implements plan of care based on individualized assessment of needs. Proficient in the diagnosis, and treatment of primary, secondary, and tertiary health conditions to ensure best possible clinical outcomes. May include formulating a comprehensive treatment and education plans for patients requiring intensive chronic disease management according to CareMore's Model of Care. Orders appropriate lab tests and diagnostic procedures and interprets results to aid in health promotion and accurate diagnosis and treatment of illnesses. Cogently explains CareMore programs and guides the patient and family toward most appropriate treatment and end-of-life decisions. Accurately documents patient's medical information and appropriately assigns the correct coding for acute, chronic, and existing health conditions. Is self-directed and takes charge of patients health needs and ensures needs are met. Effectively collaborates, coordinates, and communicates with the colleagues, primary care physicians, specialists, and outside vendors; expertly leverages all resources included in CareMore's clinical model and fosters cohesion within neighborhood team. Administers medications, injections, vaccinations, and performs minor procedures within their scope of practice. Facilitates patient transition to alternate levels of care. Evaluates nutritional needs and deficits and ensures follow-up continuity of care. Develops and researches new and innovative approaches for care and treatment of their patients. Provides leadership within the clinical setting and assists with the training and development of the MA staff including participation in teaching and evaluating clinical competence. Commendably represents CareMore in the broader community. Participates in an on-call program, which may include after-hours, weekends, and holidays. Follows evidence-based-practice guidelines. Initiates and participates in process improvement activities to ensure patient satisfaction, high quality care and judicious use of resources. Other duties and responsibilities as assigned by their supervisor to meet the needs of the organization

Qualifications

Education and/or Experience: Master's degree in Nursing with emphasis in Family, Adult, or Gerontological practice. Certificates, Licenses, Registrations: Current registered nursing license and Nurse Practitioner license in good standing with the state in which you are applying. 2-5 years of experience working in acute care settings such as ICU, CCU, Telemetry, or med-surgical unit. National Board Certification and DEA license.
