Position Purpose: The Registered Nurse is responsible for planning, directing, implementing, and evaluating all aspects of nursing care on assigned unit for residents ages 13-22. Responsible for ensuring nursing services are provided in accordance with Nursing Standards of Care and the Interdisciplinary Plan of Treatment.

The Hughes Center is a provider of Residential Treatment Services for boys and girls ages 13 to 22 diagnosed as intellectually disabled or diagnosed with any pervasive developmental disorder, such as Autism or Asperger's Syndrome.

Day School: The Hughes Educational Center is a community-based day school for male and female students, kindergarten through 12th grade.

Key Responsibilities:

1. Responsible for nursing care based on the nursing process and in compliance with facility policies and procedures.

2. Monitors, reports, and documents the residents' behavior and physiological status.

3. Administers medications and performs treatments/procedures in compliance with facility and nursing services, policies, and procedures.

4. Guides the response to residents who are behaviorally out of control or who may be experiencing a medical or behavioral crisis in collaboration with the nurse manager and physician.

5. Reports accurately and concisely to oncoming shifts regarding conditions of residents.

The nurse continuously collects data that is comprehensive, accurate, and systematic.

Units of Measure:

Completes nursing assessment within eight hours of admission.

Uses assessment as basis for nursing diagnoses and problem identification.

Encourages resident participation in data collection.

Reassesses resident as physical and/or mental condition changes.

Use SOAP format to chart by exception.

Responsible for obtaining vital signs and weight each week.

Obtain labs, document results in tracking book, file labs in chart.

Prepare documentation for outside medical appointments.

The nurse collaborates with treatment team to individualize nursing actions to meet residents' needs.

Units of Measure:

Completes admission and discharge documentation in specified time.

Focus summaries completed on time and accurate before treatment meeting.

Attend treatment meeting in absence of nurse manager.

Revises plan goals as goals are achieved or as the residents' condition changes.

Responsible for nursing care provided in the facility.

The nurse intervenes, as guided by the Master Treatment Plan, to implement nursing actions that promote, maintain, or restore physical and mental health.

Units of Measure:

Uses nursing knowledge and skills to ensure identified health care needs are met.

Employs principles of communication for interviewing techniques, problem solving, and crisis intervention when performing interventions.

Administers medications in compliance with facility and nursing services policies and procedures.

Performs treatments/procedures in compliance with facility and nursing policies and procedures.

The nurse assists residents to regain health and reduce episodes of illness through resident and family education.

Units of Measure:

Identifies resident education needs and incorporates in Master Treatment Plan, with focus on coping ADL's, socialization skills, health maintenance, communication skills, medication management, discharge teaching, and aggression reduction.

Facilitates psycho educational groups and provides health and medication teaching to families and residents.

The nurse assists in providing structure and maintaining a therapeutic environment in collaboration with the resident and other mental health care providers.

Units of Measure:

Fosters communication in the milieu consistent with therapeutic goals.

Acts as role model for staff to ensure a safe therapeutic environment.

Response to resident who is acting out will be to assist in de-escalation as needed.

Reports accurately and concisely to oncoming shifts regarding conditions of residents.

Observe and participate in physical holds.

Address medical precautions, complete physical assessment and obtain physician order within one hour after initiation of hold.

Qualifications: Good verbal and written skills, ability to rotate shifts and programs as necessary, ability to maintain confidentiality of resident information. Must consistently exhibit mental and emotional ability to understand and perform the essential functions of the job.

Education : Graduate of accredited school of nursing

Licensure/

Certification : Licensed, or licensed eligible by Commonwealth of Virginia as a Registered Nurse, CPR certification, or able to complete certification.

Experience: Prefer experience with children and adolescents in psychiatric facility or rehabilitation program.

Physical Requirements : Must be able to lift and carry up to 50 pounds. 80-90 % of the time, the incumbent is standing, walking, kneeling, reaching while working with residents. Requires full range of body motion including handling and physical restraint of residents. Ability to perform CPR and First Aid.
