Ambulatory Care Clinical Pharmacist needed for a contract opportunity with Yoh's client located in Portland, OR.


The Big Picture - Top Skills Should You Possess:



    - Experience working in Retail Pharmacy or Managed Care Environment
    - Knowledge of Managed Care and Medicare concepts

What You'll Be Doing:



    - Immediate assistance needed now through mid-August
    - Population identification of high risk medication events through state-of-the-art pharmacy risk modeling that includes how to segment populations into actionable groups and take action to assist with medication needs
    - Develop and conduct quality improvement programs related to the pharmacy program
    - Develop and implement clinical educational programs to improve drug utilization and quality
    - Develop and critically evaluate pharmacy claim data analysis/reports in support of specific projects or program objectives
    - Critically evaluate drug therapy regimens for patients enrolled in the case management program and assist with developing treatment plans
    - Provide medication therapy management (MTM) services

What You Need to Bring to the Table:



    - Graduate of an accredited pharmacy program and a licensed Pharmacist in the State of Oregon
    - Advanced pharmacy training - PharmD, residency, fellowship, board certification or master's degree in related discipline
    - Practical experience as a clinical pharmacist in pharmaceutical care management in the ambulatory care or other clinical setting
    - Previous experience in managed care and project management is preferred
    - Knowledge of the principles of managed care, pharmacy benefit management, pharmaceutical reimbursement, and pharmaceutical utilization
    - Ability to critically evaluate clinical pharmaceutical and medical literature and apply principles of evidence-based medicine
