Care Management Case Manager II needed for a direct hire opportunity with Yoh's client located in Salinas, CA.
Top Skills Should You Possess:
- Previous case management experience
- Knowledge of utilization management
What You'll Be Doing:
- Develop and manage an individualized comprehensive plan of care for members referred into the care management program with the goal of promoting optimal, achievable outcomes in the most cost effective and appropriate manner
- Perform comprehensive assessment of physical, emotional, psychosocial, and environmental needs of the member via telephonic means and/or through face to face interaction or review of relevant and available medical records
- Planning and execute appropriate interventions, evaluating outcomes and adjusting the plan as needed
- Work with and educate members, providers, external agencies and internal departments on the Care Management program and provide assistance as needed
- Participate in Quality Improvement studies, to continually evaluate the program effectiveness and ability in promoting quality driven, cost effective achievable goals and outcomes for members
What You Need to Bring to the Table:
- Current California RN license
- Associates Degree in Nursing; Bachelor's Degree preferred
- Minimum of five (5) years of experience in a patient care setting with at least one (1) year of case management experience is required
- Current working knowledge of Milliman Care Guidelines in review determination process desirable
- Knowledge of Medi-Cal and related policy and Title 22 regulations
- Knowledge of utilization management principles and activities
Bonus Points! Otherwise Known As Preferred Qualifications:
- Fluency in Spanish or Hmong is highly preferred
