Job Duties:
Communicates with patient, family, treatment team, physician and payers
Manages the utilization of services through concurrent reviews with payers and/or through internal reviews
Coordinates discharge planning activities
Completes the psychosocial assessment and reviews diagnosis.
Develops a treatment plan with specific goals delineating clinical intervention related to diagnosis for each patient
Facilitates processes and psycho-educational groups for clients and family members of clients at Medical Center of McKinney
Facilitates family discharge meetings
Rotating on-call weekend coverage of discharge planning
Communicate and document all relevant information to treatment team
May handle inquiry calls
May collect insurance and referral information
May triage patients to determine level of care (Licensed Staff Only)
May coordinate, implement and complete admission process (Licensed Staff Only)
May provide crisis assessments in med/surg hospitals (Licensed Staff Only)
Demonstrate an understanding of and adherence to the Code of Conduct
Conduct that reflects the Company's values and a commitment to the Code of Conduct
Adheres to hospital policies and procedures
Assists in maintaining a safe and therapeutic milieu.
Performs other duties as requested or required
Requirements:
Master's degree In Psychology, Counseling or Social Work from a nationally accredited school and must be a licensed provider.
Inpatient experience working with all patient populations is required; experience working with geriatric population is desired
Licensure/Certification Required:
Currently licensed by the State of Texas in Social Work (LMSW or LCSW) or LPC (LCPI and obtain LPC within one year of hire).
American Heart Association BLS required.
Must complete restraint and seclusion training as well as techniques to address aggressive or threatening behavior (as outlined in Rule 415.257 of the Texas Administrative Code) prior to assuming direct care responsibilities, if the employee does not provide proof of current certification
