Case Management RN-Part-time (.4 FTE)

The Case Management Specialist coordinates the care and services of selected patient population across the continuum of illness; promotes effective utilization; monitors health care resources; and assumes a leadership role with the interdisciplinary team to achieve optimal outcomes through the tools of case management. The Case Management Specialist will perform a wide variety of high level patient care activities to include admission assessment, screen for appropriateness, protocol monitoring, development of a plan, and patient and family teaching. Timely and appropriate discharges will be accomplished through assessment of needs and close collaboration with a social worker/discharge planner. The ability to identify opportunities for improvement in patient care and patient care support functions provided to all age groups is required.

Basic Qualifications:

Education - Graduate from a recognized School of Nursing with a current RN license to practice in the State of Michigan

Preferred Qualifications:

Experience -Three to five years acute care hospital experience is strongly preferred
