Foster Case Workers are a critical link to the community for those we serve, as well as serving as a bridge between our direct care and clinical staff to ensure the seamless delivery of services to the individuals we support.

The Hyannis Children’s Program is seeking a Foster Care Case Worker to coordinate the development and implementation of services and treatments to clients/individuals.

Foster Care Case Workers oversee all services provided to individuals and their families, monitors quality of care delivered, and monitors corporate and regulatory program compliance.

We are looking for someone with a strong sense of purpose to work in an innovative community based model. This position requires someone who will be a strong advocate for children and committed to assisting them adjust to their foster homes while working on their permanent plan.

The Foster Care Case Worker will carry a caseload of eight youth clients and their Intensive Foster parents. They partner with other agencies to support the children and families. This position also entails weekly home visits within the communities that we serve.

Qualifications:

· Bachelor’s Degree in a Social Work related field

· Experience working with children and families

· Valid driver’s license in good standing

· Car registration and vehicle insurance if providing transportation for individuals receiving services

· Successful clearance of local and/or state background checks
