SENIOR COMMUNITY HEALTH NURSE

DESCRIPTION:

The Senior Community Health Nurse is responsible for helping DOH-Charlotte County achieve its mission "To protect, promote & improve the health of all people in Florida through integrated state, county, & community efforts" and its vision "To be the Healthiest State in the Nation".

This position is based in Port Charlotte and reports to the Senior Community Health Nurse Supervisor

Job Duties and Responsibilities:

Clinical Services:

Family Planning

Educates and provides family planning services according to Family Planning protocols and TAGs

Documents health history, assessments, plan of care in the Health Management System

Documents and tracks information requested by the Healthy Start program

Encourages completion of patient satisfaction surveys

Educates and refers for tubal ligations and vasectomies according to protocol

Provides Nurse Issuance medications according to protocol and TAG

Tracks and reconciles Nurse Issuance medications

Collaborates with immunization nurse to vaccinates adults according to ACIP and CDC guidelines

Provides HIV counseling and testing

Consults with primary care providers as needed

Assists with updating Family Planning Nursing protocols and signs protocols annually

Assists with managing Family Planning contracts

Gathers data for and submits reports to Healthy Start

Serves as a backfill for Growing Strong Families and Immunization programs

Community Participation

Educates the public at various events and tracks using an outreach log

Provides sexual health education using evidence-based educational programs in a variety of community settings

Takes the lead on the Information/ Education and Outreach Committee as required by Title X funding including recruiting, organizing, and reporting

Takes initiative to increase access to family planning services

Assists with student and volunteer orientation

Other Duties:

Participates in clinical quality assurance and quality improvement initiatives

Functions as a resource to professional, paraprofessional, and support staff and students.

Represents DOH-Charlotte through participation with agencies from state, county and municipalities, and community partners.

Contributes to the success of the agency Strategic Plan

Provides other related duties in accordance with the needs of the organization and supervisor's instruction

KNOWLEDGE, SKILLS, AND ABILITIES:

Statutes, Rules and Regulations that pertain to the programs;

Use of equipment and supplies related to the position

Program development;

Nursing functions related to assessment, planning, implementation, and evaluation of services provided to family planning and STI patients

Knowledge of the prevention and treatment of infectious diseases of public health significance, prevention of diseases of environmental origin, prevention of injuries, promotion of healthy behaviors, response to disasters and assistance in the recovery, and providing access to health care.

LICENSURE / REGISTRATION / CERTIFICATION REQUIREMENTS:

Incumbent must possess a valid Florida Driver License and have a motor vehicle to carry out these duties.

Incumbents will complete mandatory trainings within required timeframes.

Incumbent must be a Registered Nurse in accordance with Chapter 464 FS; Chapter 64B9 F.A.C.

Incumbent must have current CPR/BLS or receive an approved CPR/BLS course within 90 days of employment.

OTHER JOB-RELATED REQUIREMENTS:

Incumbent may be required to work before, during and/or beyond normal work hours or days in the event of an emergency. Emergency duty required of the incumbent includes working in special needs or Red Cross shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural.
