We are seeking a knowledgeable, compassionate RN to handle patient visits on a PRN/as needed basis and will be required to travel throughout our service area to conduct the visits. As a Hospice Nurse, you would work with a team of healthcare providers to ensure our Hospice patients (prognosis of 6 months or less) are receiving the right care. Additionally, you would provide emotional support to the patients' families. This position offers potential for growth, a friendly staff, and an opportunity to interact with patients and their families on a more personal level.

 

Please note: This is a per diem, field based position. As a per diem Hospice RN you will conduct routine and/or admissions visits and also assist with PRN visits as requested. At this time, we are looking for an RN who is available to conduct visits 3 days per week (Monday-Friday) and open to working 4-8 hours per day.

    

Primary Responsibilities:


  • Conducting visits at home, skilled nursing, long term care and/or assisted living facilities

  • Coordinating, assessing, and implementing patients' care

  • Providing accurate and timely documentation of patient services to reflect plans of care

  • Conducting admissions

  • Interacting with other group members such as CNA's, LPN's, social workers and dieticians

  • Performing physical activities that require considerable use of your arms and legs and moving your whole body, such as climbing, lifting, balancing, walking, stooping, and handling of biohazard materials

Requirements

Required Qualifications:



  • RN licensure (current, unrestricted) in the state location of position

  • 3 years of clinical experience is required

  • Reliable transportation, including valid driver's license and current auto insurance

Preferred Qualifications:


  • Excellent communication skills

  • Experience in long-term care is a plus

  • Computer proficiency is a must

  • Background in home care and/or hospice care is highly preferred
