REQUIREMENTS

This position performs duties and responsibilities providing nursing services to clients being seen at the Hernando County Health Department in the Brooksville and Spring Hill Clinic(s), under the direction of the Senior Community Health Nursing Supervisor. Must be licensed as a Licensed Registered Nurse in accordance with Florida Statute 464. Must be eligible to practice nursing in accordance with Florida Administrative Code 210-8.22 or 8.27.

POSITION DESCRIPTION

This position performs high level professional duties and responsibilities in the care and treatment of patients, at and for the Hernando County Health Department. Provides and maintains continuity of health care services to clients within the clinic setting.

Uses independent nursing judgment to assess immunization records and administer needed vaccines to children and adults. Provides education and counseling on side effects, palliative measures, and follow up. Performs data entry of all immunizations given in Florida Shots ensuring accuracy and billing information. Assures vaccines are stored at appropriate temperature and maintains par levels of vaccines. Assures vaccines are stored are temperatures required for efficacy. Orients new staff on the immunization schedule and vaccines following ACIP recommendations.

Provides Pre and Post HIV (Human Immunodeficiency Virus) counseling to clients that include prevention with the educational component. Takes and records preliminary medical history, vital signs, and gives prescribed medications under protocol. Documents on the chart, utilizing problem oriented Subjective, Objective, Assessment and Plan (S.O.A.P.) method. Provides nursing assessment and diagnosis as appropriate. Provides individualized patient education in areas such as insulin administration, diabetes, asthma, and other chronic diseases. Assists clients in determining self-management goals. Interviews clients for evaluation and assessment of patient's condition, counseling and treating conditions covered under Registered Nurse protocol, and for evaluation under other Health Department programs.

Teaches lifestyles, family planning methods, and risk reduction in the Women's Health and Teen clinics and community setting. Performs limited exams prior to initiation of birth control following standing orders and refers client to provider as appropriate. Provides non-directive options counseling as needed for clients with positive pregnancy test.

Provides case management of abnormal results and specialty referrals for all Pediatric clients. Collaborates and makes referrals with other health care providers throughout the community including coordinating care with specialist in other regions of the state. Provides resource information to pediatric clients and staff. Counsels clients and families on the importance of health maintenance activities, such as routine physical exams.

Manages day-to-day clinic operations, serving as lead nurse and as a resource for clinic staff. Provides back-up to supervisor. Manages health care issues, on walk-in clients not established with HCHD/NCCHC and/or telephonic triage clients, concerning actual or potential health problems which include but are not limited to: Nursing Assessment, nursing diagnosis and triage including rapid assessment of acute physical problems when necessary, first aid and emergency treatment per protocol.

Coordinates education and clinical experience for health care students. Provides detailed orientation for new staff members.

Maintains and updates professional knowledge by utilizing educational opportunities in workshops, seminars, in-service, and participating in professional and related organizations to maintain current knowledge, skills and abilities, along with current CEUs.

Maintains security and confidentiality of all active medical records according to policy and procedure set forth by the information custodian or designee.

Responsible as a "need to know" employee for security sets relating to active medical records as it relates to performance of assigned duties and responsibilities for security and confidentiality of same.

Responsible for securing the designated information sets for the purposes of protecting confidentiality, data integrity and appropriate access, for all information, both confidential and public records, which is stored in hard copy or electronic formats?

Participates in quality improvement activities such as chart reviews and makes recommendations to enhance clinic flow and efficiency.

Performs other related duties as requested/required.

DESCRIPTION

This is work in assessing patient health problems and needs, developing and implementing nursing care plans, and maintaining medical records. Delivers infants and performs postpartum examinations and treatment.

Refers students or patients to community agencies furnishing assistance and collaborates with agencies.

Conducts specified laboratory tests.

Maintains stock of supplies.

Directs and coordinates health care programs.

Develops, negotiates and implements public/private partnerships.

Performs grant writing and/or contract management activities.

Conducts inspections or programmatic evaluations and writes reports of findings, recommendations and corrective actions.
