As a Field-Based Case Manager, you'll wear many hats, and work in a variety of environments. Sometimes, you'll interact with members leaving the hospital - possibly with new medications or diagnoses. Or perhaps you'll perform home visits, assisting members with safe, effective transitions from care environments to where they live. You may also act as an intermediary between providers and members - serving in numerous roles, such as educator, evaluator, service coordinator, community resource researcher and more. The result? Fewer hospitalizations, ER visits and costly service gaps; and a less stressed, more effective health care system for us all. Want more flexibility, want more autonomy? Primary Responsibilities:


  • Engaging members in person (in their homes) to coordinate services, community resources, and treatment needs

  • Travel in assigned territory as needed to complete member assessments; the rest of the time will be spent working in a home-based office

  • Delivering a holistic approach to coordinated care based on the member's needs

  • Identifying early risk factors and conducting ongoing assessments and documenting in an electronic file

  • Creating, reviewing and revising care plans and focusing on disease management

  • Functioning as an advocate for members and decreasing unnecessary hospitalizations

  • Collaborating with the member's PCP to deliver and coordinate necessary services

  • Building relationships with members and their families; assisting them with proper health choices

  • Ensuring cases are documented in a timely manner


 

Requirements


Required Qualifications:



  • RN license must be current, and unrestricted in state of Texas

  • 3+ years of recent experience as a RN in case manager, home health, or clinical  experience in acute, rehab or long term care setting


  • Reliable transportation to travel in assigned territory 

  • High Speed Internet access in home

  • Computer proficiency including Microsoft Word, Outlook, and Internet

Preferred Qualifications:


  • Community Based Alternatives (CBA) experience

  • Experience with Medicare or Medicaid

  • Bilingual Spanish is a plus

  • CCM certification
