We are currently seeking an LVN Service Coordinator to support Brazos County, TX and neighboring counties. Employees in this position require LVN Licensure and Certification. These roles identify, coordinate, and/or provide appropriate levels of care under the direct supervision of an RN or MD. This function is responsible for Clinical Operations and Medical Management activities across the continuum of care (assessing, planning, implementing, coordinating, monitoring, and evaluating). This includes conducting face to face functional assessments of ADLs and IDLs. This function may also be responsible for providing Health Education, Coaching and Treatment Decision support for Members. 

Primary Responsibilities:
  • Basic, structured, standard approach to work.
  • Requires current, unrestricted LPN Licensure in the State of Texas

Requirements

Required Qualifications:
  • High School Diploma/GED.
  • Current, unrestricted LVN License in the State of TX.
  • Availability for 85% Travel to homes in order to assess Member Care needs.
  • Previous experience working with a computer and Microsoft Office products.
  • Reliable transportation, a valid Driver's License, and Automobile Insurance.
  • Must reside in one of the following counties: Brazos, Milam, Robertson, Leon, Madison, Grimes, Washington, and Burleson.
Preferred Qualifications:
  • Bilingual in with English and Spanish.
  • Experience with EMR (Electronic Medical Records).
  • Home Health experience.
  • Case Management experience.
