About this job
Directs, initiates, and implements a patient care plan in a home care environment. Assists physicians during examinations and procedures. Prepares equipment, applies and changes dressings, and monitors patients. May administer prescribed medications. Relies on experience and judgment to plan and accomplish goals. Performs a variety of tasks. A certain degree of creativity and latitude is required. Typically reports to a manager or head of a unit/department.
To provide case management
functions and employee performance assessments to assure delivery of quality
health care and services consistent with regulatory compliance and agency
policies. Administers skilled nursing
care to patients requiring intermittent professional nursing services. Teaches and supervises the family, and other
members of the home health care team.
Each nurse performs nursing services for patients within a specific
area. These services are performed in
accordance with the physician's orders and under the direction and supervision
of the Clinical Supervisor or the Director.
The Case Manager performs duties
in accordance of and under the direction/supervision as defined by the Agency's
organizational chart.
Requires an associate's degree and is certified as a registered nurse. Familiar with standard concepts, practices, and procedures within a particular field.
Education: Licensed Registered
Nurse in the state of practice. BSN preferred.
Experience: Minimum of one year
clinical experience in the hospital setting, home health care nursing
experience also preferred. Six months supervisory experience preferred. Must
possess knowledge of Medicare, Medicaid, and CHAP regulations. Strong communication skills and leadership
skills required. Attention to detail and
good organizational skill needed.
Licenses/Certificates: RN license
in state of practice, current driver's license, current auto insurance, current
CPR card.
