G&K is currently seeking a Care Coordinator for an Assisted Living Facility in Culver City. As the Care Coordinator, you will be responsible for the providing overall leadership and management of the health and wellbeing of the residents within the community. Responsible for the quality assurance, regulatory compliance, coordinator of resident health and witness needs, medicals program management, and resident assessments.

RESPONSIBILITIES
Provide supervision of medications for residents. Also supervise the duties of all other medication aides.
Check to make sure all record keeping and documentation is being dine in a timely manner and neat manner
Set-up medical appointments for residents and arrange transportation to and from
Answer telephone and the intercom and take messages as needed.
Oversee the receiving, logging, and storing all medications
Contact resident's families and doctors in reference o resident status
Pass medications in dining room and residents' rooms at the prescribed times
Record keeping of medication changes and destroying of expired and/ or un-used meds
Re-ordering of medications
General assessment of residents including appetite assessment, mental and physical well-being, and to inform the Administrator of any changes in resident health or behavior
Setting up of medications per Physician's orders. Maintaining current listing of medications on the "Med Sheets" and directions on the container labels
Keep track of all narcotics
Set up new Med Sheets for each resident at the beginning of each medical cycle
Maintain current records for each resident on a medication record sheet
Account for residents that are not in the facility (ie. Out for the day, in the hospital, etc.)
Complete any additional tasks assigned by the administrator


QUALIFICATIONS
A minimum of two (2) years experience as a Registered Nurse OR an experienced LVN with supervisory experience in Assisted Living.
At least one year experience in assisted living, home health or long term care industries preferred
Demonstration of competence in assessment skills, medication administration, follow up and triage.
Minimum one (1) year nursing management experience
Must have high school diploma
Excellent communication skills in both written and verbal in English
