The Social Worker/Therapist, PRN assists in the integration of social services functions in the patient care, discharge planning and aftercare planning process with other hospital departments, external service organizations, agencies and health care facilities. The Social Worker/Therapist provides direct care to patient as appropriate and ensures compliance with quality patient care and regulatory standards. The Social Worker/Therapist assists patients and their families through the use of psychotherapeutic skills to understand, identify and resolve complex problems resulting from psychiatric illnesses utilizing group and individual therapies.

Provide psychosocial assessments. Provide individual therapy. Provides social work consultation to physicians, nurses, other hospital staff members, patients, patient's relatives and coordinates with community agencies upon requests and in coordination with assigned social worker. Participates in hospital inservice education programs to integrate social work with other hospital departments. Maintains statistical report of services provided. Attend appropriate meetings, workshops, and conferences to maintain social work skills and keep informed of community resources available. Provide discharge planning services and arrange for post-hospital care of patients through external services as requested. Co-lead group therapy on assigned hospital units. Provide clinical services as assigned. Promote positive customer relations.

EDUCATION/EXPERIENCE:

Master's Degree in Social Work or related field such as counseling, psychology or related field. Licensure by the State of Colorado (L.C.S.W.) preferred - actively pursuing a license within 90 days and license obtained within 2 years of employment.. Previous placement or internship in a psychiatric setting would be helpful, but not required.
