We are seeking an RN Case Manager Hospice to join our team of dedicated professionals as we continue our mission of improving the health of the communities we serve.

Job Description

In accordance with the Mission and Guiding Behaviors; the Hospice Registered Nurse Case Manager coordinates all Interdisciplinary aspects of Hospice care in all settings. Through assessment and reassessment functions, the Nurse identifies patient/family need s and through the interdisciplinary processes, coordinates care provided by the specific interdisciplinary personnel or providers . The Nurse evaluates patient /family response to care and coordinates needed changes to the Plan of Care. The Hospice Registered Nurse Case Manager oversees appropriate utilization of services and resources , appropriately authorizing care based on the plan of care.

requirements.

Relationship-based Care: Creates a caring and healing environment that keeps the patient

and family at the center of care throughout their experience at Mount

Carmel following the principles of our interdisciplinary care delivery system.

Develops Initial Plan of Care based on Assessment of Problems, goals, and coordinates

interdisciplinary team involvement in developing interventions.

Reviews/revises the Plan of Care as needed but at least every two (2) weeks based on

patient/family response to the plan, problems, changes in plan, need for IDT analysis, and

potential discharge or transfer.

Maintains accurate lists of patient 's current medications for over the counter and prescribed medications and newly requested orders for medication and teaches regarding safe administration on admission and reviews /revises on scheduled visits

Assesses patient status in relation to discharge criteria on each scheduled visit and

summarizes for team presentation

Reviews patient/family handbook

Other responsibilities include:

Fosters a work culture that promotes collaboration, connection, diversity and positive regard for patients/families, colleagues, volunteers and leadership.

Participates in Performance Improvement Activities through participation in clinical record reviews and current and retrospective case reviews related to high risk, high volume, problem prone, and complaint management issues

Functions as preceptor to newly hired Registered Nurses, student nurses, Medical

Students, resident trainees

Uses computer and all automated technology appropriately

Responsible for compliance with Organizational Integrity through raising questions and

promptly reporting actual or potential wrongdoing.

Qualification

* Education: Graduate from school of nursing. Baccalaureate degree in nursing preferred

* Licensure / Certification: Current license to practice in the state of Ohio

* Experience: Minimum of two years' work experience as a professional nurse, preferably

one of the two years in an acute medical surgical, home health, oncology or

hospice/palliative care setting.

* Effective Communication Skills

* Ability to operate a motor vehicle required; must possess a valid driver's license and have

access to a functional automobile during work hours.

* Demonstrate ability to assess, plan, evaluate and analyze nursing aspects of Hospice

patient/family care and work in collaboration with the Interdisciplinary team

* Demonstrated experience/ ability to provide systems oriented care to patients and families including the psychosocial, spiritual, and multicultural domains of care

* Demonstrated competence in verbal ,written, and word processing communication

* Demonstrated ability to manage time, meet scheduling targets, and function in an

autonomous manner

* Criminal background check and fingerprinting completed as required by Ohio law.
