Emergency Department Pharmacist

To accomplish these commitments, several program initiatives are underway, including: ED pharmacist and medication historian services to enhance safe entry into our hospitals, discharge medication supply and pharmacist treatment plans to ensure patients depart hospitals safely, optimal drug use in our hospitals and physician practices to support population health commitments for high risk patients, pharmacist medication management services in clinics and physician offices to help patients achieve sustained best outcomes. We're looking for motivated, enthusiastic and patient-centered pharmacists to join our regional pharmacy team and share our purpose for improving the lives of those we serve.
Located in Central Ohio, the Mount Carmel Health System serves more than a million patients each year through five hospitals and over 50 outpatient facilities.

Job Description
This position is responsible for Patient Care Pharmacist In accordance with the Mission and Guiding Behaviors the Patient Care Pharmacist, within the scope of the Ohio Pharmacist Practice Act, will provide timely, accurate and cost effective pharmaceutical care.
o Identifying gaps in care
Disease state specific medication regimens
Medication histories and allergies
Appropriate dosing based on patient specific parameters
o Assessing physician knowledge and skill related to medications
Identifying educational opportunities
Partnering to identify patient outcome initiatives and goals
Providing pharmacist consultative services for patient education
o Providing guidance on optimal treatment choices
Establishing best practice standards
Developing treatment algorithms
Participation in medical emergencies
o Improve 'time-to-treat' in high risk patient populations:
Proactive review of medication profiles
* First initiative: Focused review on 'time to treat' for antibiotics
o RN competency with medications
Mentor/Education
Review high risk medication orders: Anticoagulants, Antidotes, Insulin, Sedation
o Triaging issues
Drug information resource (easily accessible to all ED staff, including Diley Ridge, Grove City, and Delaware)
Discharge medication patient consultation (new and old medications)
Admissions to ECF's
Facilitate processing ED patient prescriptions through outpatient pharmacy (St Ann's retail pharmacy)
o Emergency/Disaster Preparedness
Clinical management of patients
Antidote or antitoxin supply management
o Regional Pharmacy Collaboration
Regional initiatives to improve pharmacy ED services
* Capturing data and results to show value of ED pharmacist
Collaboration with medication historians
Collaboration with anticoagulation centers upon admission of Mount Carmel Anticoagulation established patients.
Pharmacist Attributes:
* Highly motivated pharmacists who have excellent communication skills.
* Relationship building skills to foster a multidisciplinary approach with physicians, nurses, patient care assistants, and medication historians.
* Consistently delivering appropriate care
* Capability to assess strengths and weaknesses in leadership
* Implement day to day practice changes
* A solid background in ICU management and internal medicine.
Certifications:
* ACLS/BLS
* PALS
* NIH Stroke Certification
Hours : Evening Shift
Qualifications
* Education: Bachelor or Doctor of Pharmacy required
* Licensure / Certification: Current license to practice as a Registered Pharmacist in the State of Ohio
* Experience: PGY1 residency or One year of institutional pharmacy experience in an advanced clinical setting is preferred
* Effective Communication Skills
* Demonstrates the ability to plan, organize and manage pharmaceutical care, including delegating tasks to and providing direction to other members of the pharmacy team
* Performs work independently with minimum supervision, and is effective in a stressful environment
