About this job
Provides nursing assessment, planning and care telephonically to maximize the comfort and health of patients and families in accordance with the interdisciplinary plan of care after regular business hours and/or on weekends. Supports the Provider Relations activities of the organization.
Registered Nurse with a current, unrestricted state license, and be eligible to obtain other state licensure
Minimum of 5 years general nursing experience with current Hospice experience preferred. Experience in end of life care and pain management
Obtains CHPN certification within one year of employment if not currently certified
Ability to work weekday nights and weekends
Must be capable of maintaining regular attendance
Familiar with policies of the hospice and state and federal regulations pertaining to nursing services including Medicare certification
Commitment to company philosophy of care and service standards
Excellent assessment, nursing judgment and communication skills
Computer skills including Microsoft platform and ability to learn and utilize company EMR software
Ability to work with minimal supervision
Must be able to maintain confidentiality regarding patient, employee and company proprietary information
Must have the ability to relate professionally and positively and work cooperatively with patients, families, and other employees at all levels
