Candidates must have at least 5 years of clinical experience and at least 3 years of Case Management experience.

Assess members and establish a plan to enhance the medical appropriateness and quality of care they receive, as well as improve their overall wellness.
5-10 years clinical practice experience, e.g., hospital setting or alternative care setting such as home health or ambulatory care required. 3-5 years case management and discharge planning experience in a managed care plan, Crisis intervention skills preferred.
