About this job
Under the direct supervision of the director, the social worker is responsible for providing assessment, counseling, education, information services, discharge and home care planning services, and referrals to community resources for outpatients and their families in the cancer program. The social worker will conduct annual community assessments to ensure the needs of the patients and their families are being met, and provide community education programs, patient education classes, and support groups.
Principal accountabilities include:
Assesses the psychosocial situation of patients/families (customers) that includes financial, legal and social history in a timely and thorough manner across the continuum from diagnosis through survivorship. Completes referrals to community resources. Provides annual community resource needs assessment to identify gaps in service.
Develops, implements and maintains a distress screening program in accordance with the American College of Surgeons, Commission on Cancer. Depending on the distress screening results, provides individualized counseling as needed.
Develops, implements, and leads cancer related educational groups and support groups.
Prepares written documentation as required by the profession and by policy and procedure in a timely manner.
Works with clinic staff to develop and implement survivorship services
Licensed Clinical Social Worker or Licensed Marriage and Family Therapist.
Must have a minimum of 2 years experience in oncology specific social work.
Must have a minimum of 1 year of hospice experience.
Strong communication and problem solving skills. Strong interpersonal skills.
Knowledge of and experience with use of community services.
Must be proficient in Microsoft applications (work, excel, power point, etc.).
