**We are offering a $2500 Sign On Bonus for a Nurse with at least 2 years of dialysis OR 2 years of Case Management experience. Bonus will be paid after 90 days. **

FACT: As of August 2013, Patient Pathways has served over 160,000 renal patients from coast to coast, empowering them to be more successful in their healthcare journey while reducing hospital length of stay and re-admissions.

This key role is based in Valley Hospital and focuses on patients living in this area. This position functions autonomously and in collaboration with all members of the healthcare team to coordinate and facilitate quality, cost-effective care, while minimizing fragmentation of the healthcare delivery system for ESRD and CKD patients. This position provides a link between the patient, family, physician, provider and care teams, the community, and Regional Operations Manager.

Care Management Nurses are required to: Identify and assist with the needs of patients including, but not limited to medical, social, emotional and financial concerns; implement innovative nursing interventions appropriate to the long-term needs of the patient; use relevant data and information obtained through the interview process to coordinate care; identify and redirect practice patterns when necessary; as well as identify existing and required clinical services in each market area and other tasks as needed.

Qualifications

A.D.N degree from accredited school of nursing required; B.S.N preferred; three-year diploma from accredited diploma program may be substituted for nursing degree

Current RN license required; continuing education credits maintained as required by state of practice

Certified Nephrology Nurse (CNN) or Certified Case Manager (CCM) preferred.

Experience with Case Management

Previous dialysis preferred

Current CPR certification required
