Summary: The Homecare Social Worker is responsible as a member of the interdisciplinary team for the provision of psychosocial assessment, planning and care in order to maximize the comfort and health of patients and families consistent with the Kindred at Home policies and procedures. The Homecare Social Worker is responsible for identifying patient/family psychosocial needs and for providing supportive care and appropriate services in accordance with the attending physician's orders and the interdisciplinary team plan of care.

Qualifications:

Education : Graduate of an accredited Bachelor's Degree in Social Work program. Masters Degree in Social Work preferred.

Licenses/Certification : A valid Social Worker license in the state employed is required as is current CPR certification. A current driver's license is required. Reliable transportation and current auto insurance is required.

Experience: Minimum of 2 years experience as a professional social worker and preferably one year in a home health setting. Demonstrated ability to assess and respond to the needs of patients and families in varied settings. Demonstrated ability to work independently and cooperatively as an effective team member. Ability to communicate effectively with patients and their family members, and at all levels of the organization. Ability to maintain confidentiality.
