As a Clinical Pharmacist you will maintain or enhance clinical pharmacy programs such as health outcomes, disease state management, care management, and other specialty programs. Primary Responsibilities:
Performing MTM telephonic pharmacotherapy consultations to patients and/or prescribers promoting high quality, evidence-based, cost effective pharmaceutical care
Completing a comprehensive medication review of prescription and non-prescription medications with emphasis on patient assessment and counseling, safe and effective use of medications, identification of potential drug interactions, and other related activities
Completing pharmacotherapy assessment of patients' claims, past medical history and disease states
Assessing patients for medication adherence barriers and develops a plan to maximize patients' medication success through improving or maintaining adherence to medications
Developing a plan of care that details actions of the patient and/or prescriber based on the Pharmacotherapy assessment
Delivering high quality evidence-based therapeutic recommendations to providers
Documentation of clinical consultations including the plan of care into internal data base
Participation in continuing education sessions provided by the department
Effectively participates in and contributes to staff meetings and other staff development opportunities and interdepartmental work groups
Identifies areas of improvement where systems modifications could result in operational advancements and efficiencies
Maintains a high level of clinical knowledge of chronic disease states and changes in pharmacy practice in adult and geriatric patient populations to serve as an authoritative information source on drugs and their utilization in therapy

Requirements
Required Qualifications:
Pharmacy degree, and active, unrestricted license to practice as a Pharmacist in the state of Florida
1+ years experience as licensed Pharmacist (any setting)
Comfortable with change management and matrix environment
Strong communication skills (both verbal and written)
Strategic vision with attention to detail
Able to interact effectively with various departments and levels of management
Must live within a commutable distance of our Miramar, FL office location
Additional Preferred Assets:
Pharm D
Completion of PGY1 and/or PGY2 pharmacy residency
Experience in managed care pharmacy or health plan
Experience in MTM (Medication Therapy Management)
Medication Therapy Management Certification
Experience working with the geriatric patient population
Bilingual skills (fluency in Spanish, Cantonese, Mandarin, or Russian)
