POSITION SUMMARY:

The Medical Social Worker provides crisis intervention, short term counseling, information and referrals for patients and their families, ranging in age from infants through geriatrics. Provides coordination of complex discharge planning needs and concrete service needs of patients. Leads educational and support groups. Provides consultation and training on psychosocial aspects of care as a member of the interdisciplinary treatment team. Serves as an expert about and as a liaison to community resource programs. Participates as a member of the Bioethics Committee.

As Case Manager, performs and coordinates all aspects of patient case management involving integrated discharge planning and utilization review to ensure effective, high quality and cost-efficient results. Achieves a timely, effective coordinated case management effort by actively involving members of the healthcare team. Uses effective interpersonal skills in managing the complexity of interactions involved with this position. Education/Licensure:

1. Master's degree from a program accredited by the Council on Social Work Education.

2. Current California licensure as a "licensed clinical social worker" preffered. Demonstrated with MSW acceptable.

Experience:

1. Two years experience in a healthcare setting; one year acute care hospital experience. Experience in crisis intervention; short term counseling, coordination of complex discharge planning needs, leading support groups.

2. Previous experience in utilization review or discharge planning in an acute care setting highly preferred.

Knowledge:

1. Strong clinical psychosocial assessment skills, with the ability to conceptualize clinical situations and demonstrate application of theory, appropriate to specific age groups, in psychotherapeutic and case management activities.

2. Knowledge of child, elder, domestic, and dependent adult abuse reporting requirements and other significant regulations affecting medical social work practice.

3. Knowledge and understanding of individual, child and family development; human behavior and communication; psychotherapy; case management; and the influence of culture and values in social work practice.

4. Broad knowledge of community resources, Medicare and Medi-Cal regulations, contract regulations, and legal policies is required.

Special Skills/Equipment:

1. Demonstrated ability to effectively communicate verbally, and in writing.

2. Must demonstrate strong interpersonal and organizational skills, to work effectively in a crisis-laden environment under significant pressure.

3. Must be available for scheduled coverage and flexing of work hours, as necessary.
