Summary: The Nurse Case Manager in Hospice is accountable for coordinating patient care with the interdisciplinary team, physician, patient/family and referring agency, administer skilled nursing care for clients of all ages in their place of residence and assume responsibility for coordination of care.

Education: Graduate of an accredited Diploma, Associate or Bachelor's School of Nursing .
Licenses/Certification: Current state license as a Registered Nurse. Proof of current CPR, Hepatitis profile and TB vaccination record. Current driver's license. Must have reliable transportation and insurance.
Experience: Two years experience as a Registered Nurse in a clinical care setting, home health or hospice preferred.
