About this job

Utilizes clinical judgment and expertise to coordinate care management activities and develop strategies to ensure cost effective, quality care outcomes to a caseload high in complexity and acuity. Identifies options for levels of care for individuals and populations of patients, focusing on strategies that will promote optimal health. Assesses, plans, and provides therapeutic interventions that enhance psychosocial adjustment to illness/injury, facilitates seamless care for patients and ensures effective and timely use of community resources to address identified needs. Provides payers with accurate and updated medical information. Promotes quality care delivery across the continuum of care. Facilitates informed, appropriate, and timely patient discharges. Works with other members of the health care team (both inpatient and outpatient) to advocate for and ensure appropriate aftercare and follow up. Emphasis of duties may vary depending on area of assignment.

Basic Qualifications:

Experience - 1 year of experience in Requirement for health care related experience is department-specific (ie, home health, long term care)

Licenses - Licensed, or be eligible to pursue, a State of Michigan Social Work License\\Upon Hire\

State of Michigan Social Work License\\180 Days\

Preferred Qualifications:

Education - Master's Degree in Social Work; exceptions for persons without a social work degree can be made for those who have a human services degree and carry a current social work certification that was grandfathered prior to July 1, 2005

Experience - 1 year of experience in community resources and discharge planning,1 year of experience in department-specific (eg, mental health, inpatient, etc)
