Providence is calling a Social Worker - Population Health to Providence Medical Group in Spokane, WA.

This is a Full Time (FTE = 1.00), 1st shift position.

We are seeking a talented caregiver to join our team and work collaboratively with the team to support our commitment to high quality value-based care as we are undergoing rapid growth in our risk-based payment arrangements. This position operates in a highly autonomous manner and utilizes a diverse set of problem solving skills, navigating multiple databases to obtain the necessary documentation and attain quality metrics.

In this position you will:

Pull ta and reports from multiple databases and electronic health records to identify potential care gaps and identify appropriate process to close gap(s).

Work collaboratively with clinics throughout the community to obtain necessary documentation to close identified care gaps.

Develop work flows and processes to attain targets established by the Health System and value-based contracts.

Perform outreach to patients on behalf of the clinic in accordance with their policies and procedures.

Work with payors to obtain monthly reports and abide by their protocols for closing care gaps.

Develop reporting mechanism to provide dashboard reports for quality tracking. Meet periodically with the Director to review value-based contracts and their quality targets.

Work collaboratively with clinic staff, physicians and leadership to present performance opportunities.

Utilize Office suite applications, EMR, and data management techniques to obtain appropriate level of detail for solving complex problems.

Validate patients have received identified services and ensures appropriate documentation completion.

Apply knowledge of practice policies and procedures related to scheduling and front desk functions.

Scan and fax as required by clinic and payors.

Answer multi line phone system to clinic standards, including proper chart documentation.

Required qualifications for this position include:

Certified Medical Assistant.

Current Basic Life Saving (BLS) certification through the American Heart Association (AHA) required within 90 days of hire.

Customer service related experience.

Preferred qualifications for this position include:

Associates Degree - Health Care related field.

One (1) year recent experience in the medical field.

Knowledge of medical insurance.

Analytical, interpersonal skills, Electronic Medical Record and HEDIS quality metric experience.
