Position Description:The Practice Support Liaison (PSL) is the C&S solution to carry out STARS focused strategies aimed at improving provider outcomes through direct intervention at the practice level. The PSL position is driven by the necessity for a local presence in the C&S market to drive STARS performance to the next level as well as assess and improve quality metrics and overall patient outcomes. This model is driven by directly imbedding the PSL into the provider office, providing a direct link between UHC membership and our key providers.Research member-related health information (e.g., medications, clinical records, policies, benefit information) using internal and external systemsUse desk top computer applications (e.g., Excel, Word, Outlook, Office Communicator) to summarize information (e.g., reports) or communicate member information to others.Document information (e.g., assessment information, member interactions, referrals, follow up plans) in relevant computer systems.Navigate multiple applications at once to obtain and/or document member informationLearn updates and changes to computer systems/applications and apply to navigate applications effectivelyExecute and meet performance goals within a virtual environmentNavigate Virtual Call Center (VCC) system to receive inbound calls, make outbound calls, and document call time and activity appropriatelyAssistance with HQPAF completion through EMR review and uploading of HQPAF forms with supplemental data to Optum

Requirements

Minimum Requirements:HS Diploma/GEDLPN license in the state of TNMust be able to travel between provider offices within the areaProficient with Microsoft OfficeMust have a valid driver's license within the state of TN6+ months of Medicare and/or Medicaid experience6+ months of Customer Service ExperiencePreferred Qualifications:EMR experienceHEDIS knowledgeCareers at UnitedHealthcare Community & State.
