As a Field-Based Case Manager, you'll wear many hats, and work in a variety of environments. Sometimes, you'll interact with members leaving the hospital - possibly with new medications or diagnoses. Or perhaps you'll perform home visits, assisting members with safe, effective transitions from care environments to where they live. You may also act as an intermediary between providers and members - serving in numerous roles, such as educator, evaluator, service coordinator, community resource researcher and more. The result? Monday-Friday position with work hours of 11:30 am to 8 pm.
 
Primary Responsibilities:

Contacting referred members and introducing the program
Managing members through their pregnancy and six weeks after birth
Identifying gaps or barriers in treatment plans
Providing patient education to assist with self-management
Interacting with Medical Directors on challenging cases
Coordinating care for members
Making referrals to outside sources
Coordinating services as needed (home health, DME, etc)
Educating members on pregnancy
Encouraging members to make healthy lifestyle changes
Document and track findings
This position will require active and unrestricted Nursing licensure in multiple US States. Selected candidate must be willing and able to obtain and maintain multiple state licensure. (Application fees and filing costs paid for by UHG)

Required Qualifications:

Current, unrestricted RN license in the State of New York
3+ years experience in a hospital setting, acute care, direct care experience or experience as a telephonic Case Manager for an insurance company
Minimum 2 years of maternity nursing experience
Multiple state licensure (in addition to Compact License if applicable) or ability to obtain multiple state nursing licenses
Basic level of experience with Microsoft Word, with the ability to navigate a Windows environment
Preferred Qualifications:

Bachelor's Degree in Nursing (BSN)
Background in managed care or prior Case Management experience
Certified Case Manager (CCM)
5+ years of Maternity nursing experience
Experience with high risk pregnancies
Labor & Delivery (L&D) experience
