Primary Responsibilities:

Providing preventative and primary care for patients in assigned long term care facilities and skilled nursing facilities
As a Per Diem  Nurse Practitioner you will perform proactive and acuity visits to patients in the Columbus area- long term care facilities and skilled nursing facilities during the week; covers for vacation as needed and available
Working with primary care physicians to provide the best care possible
Collaborating with the nursing staff and the patients' families
An opportunity to learn palliative care is offered with Optum Complex Care
Working with the facility's utilization review process

Requirements

Required Qualifications:

Current, unrestricted NP or CNS license in the State of Ohio (or reciprocity)
DEA License or ability to obtain
Nurse Practitioner or CNS national certification as ANP, FNP, or GNP required
1 year of NP/CNS experience
A clinical background in adult, family or geriatric specialties 
Past experience working in a nursing home or with seniors in other settings
Intermediate level of proficiency in PC based word processing including Microsoft Word and Outlook
Valid driver's license, reliable transportation, and current automobile insurance
