According to the plan for care prescribed by the Registered Nurse for assigned patients, the LPN will provide individualized nursing care in accordance with the Standards of Nursing Practice for adult, geriatric and pediatric patients.
Responsibilities:

Collaborates in the assessment of patient/significant other in a timely, consistent and systematic manner. Documents subjective and objective data according to policy.
Collaborates in the development of an outcome oriented plan of care that reflects the patient's actual or potential health problems.
Implements the plan of care under the direction of the RN to achieve the identified outcomes for the patient according to policy and procedure.
Assists in evaluating the effectiveness of services provided.
Demonstrates effective and accurate verbal, written and electronic communication.
Participates in unit and organizational activities.
Demonstrates effective patient safety techniques.
Demonstrates continued development of knowledge and skills.
