Position summary

Manager, Care Coordination (MCC) leads professional associates and operations of CHN's geographically-broad care coordination program. Based on CHN strategies and priorities established by the CHN Board of Directors, works with physician leaders and managers to develop and implement locality-appropriate operational programs that facilitate effective care coordination across patient teams with specific focus on population management and ambulatory patient case planning. These programs enable the right patient services, at the right time and place, reinforcing wellness by augmenting medical treatments with care coordination consisting of timely assessments, staging and orchestrated transitions across care sites and teams. Will identify and sustain important patient-centric entities and relationships across the delivery system and community. Directly drives resource stewardship, applies evidence-based decision support tools and applications to eliminate medically-unnecessary and duplicative services. Provides formal leadership and supports system transformation to value-based services, as well as mentoring of physician leaders, direct reporting associates, colleagues and others.

education requirements

Bachelors Degree in Nursing

work experience requirements/Preferences

5-7 years in recent active RN practice, preferably supervising others and caring for patients with complex or chronic conditions

5 years clinical management leadership experience, preferably in multi-site ambulatory sector requiring oversight of operaitional programs and professional staff engaged in complex care activities

Prefer experience in managed care, population health, case/care management, utilization management or chronic disease management environment

License/certifications

Current Colorado RN license in good standing

Prefer certification by relevant professional organization
