Position:  Registered Nurse
Reports to:  Manager, Client Services
 
 
Responsibilities:
 
Assessments, Reassessments, Quality Visits, Training:
  • Perform initial and ongoing client assessments of Personal Care clients to determine the appropriate level of service.
  • Initiate, prepare, implement, and evaluate the care plan; make documented revisions to the Plan of Care as necessary.
  • Prepare and maintain clinical documentation including nursing assessments, Plan of Care’s, and discharge summaries for all clients receiving Personal Care.
  • Review and communicate change in the client’s condition or needs to the Manager, Client Services.
  • Make a supervisory visit to each Personal Care client at least every 60 days to assess the client, the Personal Care caregiver’s delivery of care, and determine whether client outcomes are being met.  Document findings.
  • Conduct quality assurance visits with Companion Care and Personal Care clients.
  • Supervise and train Personal Care caregivers.
  • Help develop and implement state-of-the-art training programs for Personal Care caregivers.
 
Qualifications: 
●   Maryland RN License issued by the Maryland Board of Nursing
         ●   Provide proof of negative tuberculosis test prior to first assignment and annually thereafter
         ●   Provide proof of Hepatitis B vaccine or sign Declination Statement prior to first assignment
         ●   Provide proof of current CPR training
●   Geriatric, long-term care, or case management experience preferred, not required
●   Education, knowledge, or experience working with clients who have dementia/Alzheimer’s or cognitive
     challenges preferred
●   Successfully pass a background check
●   Be able to speak and hear English clearly
●   Be able to read and write English on an average adult level
●   Ability to drive with current valid driver’s license, and vehicle insurance to work site locations
●   Ability to adapt to various living environments and locations
●   Ability to lift up to 25 pounds, bending, twisting, stopping, kneeling, reaching, pushing, pulling, and carrying
●   Ability to withstand periodic contact with dog and cat hair, dust, mold, mildew, and cleaning solutions.
