Medical Social Worker-Care Coordination* - Full-time / Part-time

As a member of the Clinical Resource Management (CRM) team, the Social Worker performs psychosocial assessment to determine the patient's prior level of functioning, expectations and concerns which may affect his/her ability to adjust to illness or disability, ability to benefit from medical treatment and/or transition to the community. The Social Worker coordinates, plans, implements, and evaluates the discharge plan for each patient based upon initial and ongoing psychosocial assessments in collaboration with the multidisciplinary team. Plans and performs interventions based upon the assessment findings, including education, counseling, advocacy and referral. Provides clinical interventions with patients and their families, including crisis intervention, anticipatory grief work and supportive counseling.

Masters degree (MSW) from accredited school of social work required

Current licensure as a Licensed Clinical Social Worker (LCSW) in the State of California preferred

Minimum one year clinical experience required

One year medical social work experience (utilization management, discharge planning)preferred

Knowledge in health care financial environment, and requirements and eligibility parameters for all payor types is needed. Data entry and retrieval (MIDAS), and other basic computer skills are necessary
