Summary: Administers skilled nursing care to patients requiring intermittent professional nursing services. Teaches and supervises the family, and other members of the nursing team. Each staff nurse performs nursing services for residents within a specific area.

Essential Duties and Responsibilities include the following. Other duties may be assigned.

Monitoring through observations: patient, staff and physician feedback; written and verbal communication; quality improvement/quality assurance activities; records management.

Responsible for other Registered Nurses, Licensed Practical Nurses, and Home Health Aides who may be working on a particular home care case for which the primary RN is responsible for managing.

Responsible to communicating with all disciplines involved in the patient's care(i.e.: Physical Therapy, Occupational Therapy, Speech Therapy..)

The RN performs duties in accordance of and under the direction/supervision as defined by the Agency's organizational chart.

Provides services requiring substantial knowledge and skills in accordance with the plan of care, care plan and standards of professional practice.

Performs therapeutic procedures and treatments as ordered by the physician.

Initiates and applies preventive and rehabilitative care techniques.

Uses a systematic approach to plan, assess, implement, and evaluate consistency between the plan of care and the careplan.

Communicates changes in the patient's condition to the team leader, physician and the other team members.

Maintains a liaison role between the company, patient, physician, health care team members, community services, and organizations to assure effective communications, coordination of care, and supervision of skilled/nonskilled staff.

Participated in case conferences and acts as a resource to other health team members in the identification and resolution of patient needs.

Maintains and submits documentation, including clinical and progress notes, as required by the Agency.

Maintains confidentiality regarding patients, health care staff, and documentation.

Participates as a member of company committee, community organizations, and professional associations.

Performs additional duties as assigned by the Supervisor or the Director.

Supervises LPN/LVNs and paraprofessionals providing services and assigns Home Health Aide to specific patient.

Attends Community Care in-services and education as required.

Adheres to Federal/State guidelines, rules, and regulations, statues and laws.

Follows the Code of Conduct Handbook.

Complies with corporate and departmental policies and procedures.

Provides training and directs staff as appropriate. Attends and participates in staff meetings, in-service programs, and utilization reviews when appropriate. Provides orientation programs as assigned.

Reasoning Ability:

Ability to define problems, collect data, establishes facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.

Computer Skills:

To perform this job successfully, an individual should have knowledge of Word Processing software; Spreadsheet software and Database software. Must be proficient in the use of a PC and Microsoft Office Suite (Access, Excel, Outlook, Power Point and Word)

Physical Demands:

Occasionally(up to 33%)- Climbing, Pushing up to 300 lbs.

Frequently(34-66%)- Squatting, Kneeling, Twisting, Lifting 50 lbs, Carrying 50 lbs, Driving.

Continously(67-100%)- Sitting, Walking, Standing, Bending, Lifting up to 50 lbs, Carrying up to 50 lbs.

Visual and Hearing Requirements:

Must be able to see with corrective eye wear, Must be able to hear clearly with assistance, Must have a clear speaking voice.

Works as schedules. Notifies Supervisor in a timely manner of any absence from work. Supports the Mission and Vision of the Agency. Supports the team effort of the Agency. Treats all agency employees with dignity and respect. Responds promptly, accurately, courteously, and empathetically to provide information, address requests, problems or concerns of all customers including patients, their families, staff, physicians, vendors, and other healthcare personnel. Follows agency dress code and policy. Wears identification badges at all times while at work. Qualifications:

Education: Licensed Registered Nurse in the state of practice.

Experience: Minimum of one year clinical experience in the hospital setting and two years home healthcare nursing experience preferred.

Licenses/Certifications: RN license in the state of practice, current driver's license, current auto insurance, current CPR card.
